Mail-in Form Demonstration Script

This is a demonstration page (not in beautiful HTML) which shows how you can get information very simply if all you want is to have it mailed to you.

The HTML on this page uses "hidden" attributes to specify who the form will be mailed to (that's on the POST attribute), what the subject of the email message will be (that's the SUBJECT entry), and what will be sent back to the user if the form could be mailed successfully (that's the SUCCESS entry). Otherwise, this simply sends mail with all of the entries nicely formatted.
