FillOutForm script documentation

Filloutform is a script which simply repeats back all the fields in a form. The general idea is that if you have a form you want someone to fill out---but you don't want them to mail it in---you can use filloutform to fill out the form and then tell the user to print the form and send it in.

There are two "magic" fields in filloutform. They are "before" and "after." These should translate to URL-like filenames (e.g., something like /www/testing/foobar.text and not http://www.opus1.com/www/testing/foobar.text) without the scheme and domain name. When the script is executed, these will be included, without change, before and after the form text. You should put any HTML code you want into those files.

Filloutform will send to the client the before file (if one is present), a horizontal rule, then each field in the form, in the order that they were sent to the server, a second horizontal rule, and the after file (if present).
